Abstract

Background: Thrombocytopenia is a very frequent problem among sick neonates, affecting up to 35% of all infants admitted to the NICU. Although multiple clinical conditions have been causally associated with neonatal thrombocytopenia, the cause of the thrombocytopenia is unclear in up to 60% of affected neonates and the risk of developing thrombocytopenia is inversely proportional to gestational age (GA)
Aim of the Work: Primary aim: to compare the effectiveness of liberal and restricted platelet transfusion practices in thrombocytopenic neonates admitted to NICU of Ain Shams University hospital in reduction of major bleeding, duration of NICU admition and mortality. Secondary aim: to study predictors of major bleeding in neonates.
Subjects and Methods: This cohort study included (50) neonates with severe thrombocytopenia (27 males and 23 females) with mean GA 31.9 ± 3.4 weeks, admitted in NICU of Ain Shams University Children’s Hospital over a period of 12 months from April/2017 to April/2018. This study was approved by Ain Shams University ethical committee. Informed consents were taken from parents or legal guardians after full description of the study protocol.
Results: This prospective study was conducted in Ain Shams University NICU over a period of 12 months from April 2017 to April 2018 on 50 neonates with platelet count below 50×109/L. Fifty four (27) received restricted platelets transfusion while forty six(23) neonates received liberal platelets transfusion.
Conclusion: Prematurity and neonatal sepsis are the most common causes of thrombocytopenia. Most cases of major and severe bleeding occurred in neonates with gestational ages below 28 weeks. Lower platelet nadir was not associated with increased incidence of major bleeding.
