Abstract

Summary
The current study was carried out in Department of
Anaesthesia, General surgical unit, in Tanta University Hospital, on
100 patients, ASA physical status I & II scheduled for elective
haemorrhoidectomy under saddle block with pudendal nerve block as
postoperativeanalgesia. Patients were randomly allocated into two
groups, group I(received 5ml bupivacaine 0.5% with 5ml normal
saline each side in the block)& group II (given5ml bupivacaine0.5%
with 5ml normal saline containing ketamine 0.5mg/kg each side in
the block) 50 patients each. After approval from the Local Ethical
Committee and all patients had informed written consent of the study.
All patients divided into two equal groups patients each
according to anaesthesia induction:
A spinal anesthesia with 1.0 mL hyperbaric bupivacaine
0.5% with a 25-gauge Whitacre pencil-point needle inserted between
the lumbar intervertebral space 4/5 under aseptic conditions.
 group I: received Ultrasound guided pudendal nerve block
on both sides using 10 ml volume each side (5 ml bupivacaine
0.5% and 5 ml normal saline ) after saddle block.
 group II: Received Ultrasound guided pudendal nerve block
on both sides using 10 ml volume each side (5 ml bupivacaine
0.5% and 5 ml normal saline containg ketamine 0.5mg/kg)
after saddle block.
