Abstract

Type 2 diabetes is a growing worldwide public health problem in both developed and developing countries, with the number of patients in the world expected to increase from 171 million in 2000 to 366 million in 2030. In Egypt, Type 2 diabetes is also a major health care problem with estimated prevalence of 15.56% in year 2013 in the population of 20 years of age and older.
The present study was conducted in Desuq primary health care newly accredited units in the past 3 years .Medical records of diabetic patients who visited the selected units during the period from Jan 2015 to Dec2015
The aim of the present study was:
To study clinicalguideline adherence and health outcomes in diabetes mellitus type 2 patients in accredited primary health care units in Desuq, Egypt.
To assess the guideline adherence of diabetic patients to diabetes care in accredited primary health care units.
To assess patients health outcomes of process of diabetes care after one year of follow up.
To determine association between guideline adherence of diabetic patients and health outcomes.
Descriptive longitudinal study was carried out on 300 diabetic type 2 patients and the following results were obtained:
The age of the patients ranged from 30 to 65 years (mean 48.4 ± 6.46 years). About half of them (48.7%) ranged from 40 to less than 50 years old, 12.6%ranged from 30 to less than 40 years and 38.7% were 50+ years .About two third of the sample were females and 89 % of them were married.Regarding level of education 87.7% were illiterate &primary education and 12.3% had secondary &university. Among the study sample 58.3% were not working.Regarding income level 87.7% did not have enough incomeYears since diagnosis, ranged from 1 to 12 years (mean 1.8±2.02), where 62% were diagnosed from 1 to less than 5 years, 32.7% from 5 to less than 10 years followed by 5.3% since 10 years and more. About three quarters 77.0%of the study sample was taking oral medication, while 12.0% used insulin injection.
It is clear that guideline adherence varied between processes. It was very high (about 90%) in one half of the 14 process measures , about 50% (ranging from 55% to 69%) in 6 out of 14 process measures, while in only one measure it was below 50% .
Achievement of recommended targets for major outcome measures showed that the proportion meeting targets for outcome measures varied between variables. High percentage were noted as regard BP (76.7%) and serum creatinine (80.9%), while lower percentage were found for FBS (42%) and HBA1c (45.1%). As regards BMI it was only 6.7%.
