Abstract Tiny (very small) intracranial aneurysms generally considered to be those of 3-mm diameter or smaller. This group of aneurysms was not included in the International Subarachnoid Aneurysm Trial (Molyneux et al. 2005) because coiling of such tiny aneurysms was considered to be technically challenging due to the higher procedural perforation rate and the relatively wide aneurysm neck. (Gupta et al. 2009). The study has been conducted on patients presented with Subarachnoid Hemorrhage from October 2015 to April 2019 in Menoufia University, Alexandria University and private hospitals. They were investigated with digital subtracting angiography (DSA) and three Dimensional Rotational Angiography (3DRA) to study the morphology of the aneurysms. Thirty-eight patients with ruptured tiny aneurysms with ≤3 mm in size were investigated in our study, which measures ≤3 mm in at least one of the dimensions. Twenty-eight patients treated with endovascular coil embolization to study its feasibility and durability. Ten patients treated with surgical clipping were added to study the cost effectiveness of coiling of the tiny cerebral aneurysms in comparison to clipping of the same aneurysms size. Clinical follow-up was done in most of the patients directly after the intervention and for mean of period ≈ 199 days later using WFNS scale. Radiological follow-up also was done after the intervention using either CT Angiography (CTA), MR Angiography (MRA) or Digital Subtraction Angiography (DSA). |
