Abstract

Autoimmune diseases,including RA,SLE and PsA , affect a large number of people in whom one of the leading causes of morbitity and mortality is cardiovascular disease.
Increased cardiovascular morbidity and mortality in patients with PsA cannot be entirely explained by traditional risk factors, suggesting that non-traditional cardiovascular risk factors as PsA-specific CV risk factors or inflammatory CV risk factors that characterize these diseases may accelerate atherosclerosis .
The aim of this study was to detect the presence of premature atherosclerosis in psoriatic arthritis patients , to measure level of serum leptin and to detect its association with the presence of premature atherosclerosis in this patients. .
The following was done in a total of 45 PsA patients and 45 healthy subjects as control group:
 Patients were subjected to history taking and clinical examination to determine the clinical type of psoriasis, distribution, involved BSA.PASI score and DAPSA score .
 Routine evaluation of CBC , renal function , liver enzymes as well as ESR , CRP and RF.
 Evaluation of BMI , fasting insulin ,fasting blood glucose to calculate HOMA-IR, plasma lipid profile and serum leptin level.
 Carotid Doppler to determine CIMT.
The results in this study showed that:
 There was no statistical significant difference between the two groups regarding age and sex.
 There was no statistical significant difference between the two groups regarding BMI and waist circumference.
 T.cholesterol,LDL,and TG were higher in patients than controls.
 HDL –C was lower in patients than controls.
 Serum leptin was higher in patients than controls (p=0.00001).
 HOMA-IR was higher in patients than controls.
 CIMT increased in patients than controls(p=.011) and there were 14 patients had plaques but no one in controls had plaques.
 There was significant correlation between serum leptin and disease duration ,BSA ,PASI,DAPSA , DAS 28 and CIMT.
 There was no statistical significant correlation between serum leptin and laboratory variables (FBG, HA1C, HOMA IR, F.Insulin ,WBC and Hb).
 There was no statistical significant correlation between serum leptin and lipid profile.
 There was no statistical significant correlation between serum leptin and liver enzymes, RF, ESR, CRP, Urea, S.creatinine and uric acid.
 CIMT has significant correlation with HDL,LDL,T-cholesterol, HOMA-IR,CRP,DAS 28 and serum leptin.
 There was no statistical significant correlation between CIMT and demographic data ( Age , BMI and waist circumference ).
 There was no statistical significant correlation between CIMT and duration of the disease.
 There was no statistical significant correlation between CIMT and BSA score , PASI
