Abstract

Ovarian cancer is the fifth most common cause of cancer death in women behind lung, breast, colorectal, and pancreatic cancers, accounting for more than 3% of all cancers in women and causing more deaths than any other gynecologic malignancy.
Most ovarian cancer typically presents late, stage III-IV, after the disease has spread widely out of the pelvis.
Our study was applied to 50 women for staging of ovarian cancer. MDCT was performed for all patients using 16 multidetectors system and 43 cases underwent surgical biobsy with subsequent surgical staging and evaluated by an experienced pathologist and the other 7 cases were stage IV by CT staging and did not undergo surgical staging and were excluded from the study, the mean age was 50-70years.
Abdominal discomfort was the most common complain, half of the cases were unilateral and the other half were bilateral, most of the cases were solid lesions (23cases) and the malignant epithelial ovarian tumor were the most common tumor in our study(35cases) .
The sensitivity, specificity, positive and negative predictive value of MDCT in detection of LNs involvement, peritoneum and omental deposits were calculated using standard statistical formula in comparison with surgical and histo-pathological findings, MDCT detected most of the cases with lymph nodes involvement (20 out of 24),MDCT detected most of the cases with peritoneal nodules (9 out of 13) and all cases of omental deposits(12 cases)
Most of the cases (26 cases represent 60%) were present in higher stages III and 17 cases were in stage I and II.
In our study the accuracy of MDCT in staging ovarian carcinoma is higher in advanced stages (III) than in early stages (I,II),this is due to the high MDCT capability to discover peritoneal seeding and abdominal visceral organs involvement. The overall diagnostic accuracy of MDCT in staging of ovarian cancer in our study was 90%
Finally, our study concluded that however the possibilities of over-staging and under staging of ovarian cancer, MDCT inform excellent information on detection, determination of the possibilities of tumor resection, staging of ovarian cancer which is more accurate allowing for better planning for the right treatment approach.
