Abstract

This study was carried out on a total of 40 patients with oral pemphigus vulgaris. The patients were diagnosed by typical clinical finding & typical histopathological finding. The patients were able to read and give consent. Patients with concomitant oral disease or infection (candida infection –dental malformations) or systemic disease that can interfere with wound healing process were excluded. Patients were selected from out-patient clinic of dermatology, venereology and andrology department, Mansoura University hospitals in the period between December 2017 and December 2018. The forty patients were divided to 2 equal groups: GroupA (20 cases) injected with autologous platelet rich plasma every 2 weeks for 3 months. GroupB (20cases) injected with triamcinolone acetonide (10 mg/ml) every 2 weeks for 3 months. All patients were subjected to complete medical history, dermatological examination and documented digital photography. In this study, there is no significant difference regarding the pain of the disease or side effects between autologous PRP and ILS in the treatment of oral erosions of PV, however, Regarding the response to triamcinolone acetonide, six patients (30%) had marked improvement, six patients (30%) had moderate improvement and eight patients (40%) had mild improvement compared to the response to platelet rich plasma, ten patients (50%) had marked improvement; ten patients (50%) had moderate improvement In our study there is statistically highly significant difference between PRP group and steroid group regarding clinical improvement of the oral lesions and this makes our study better than previous studies and also we have larger number of patients and this is in the good of the study. In conclusion, based on our findings, Platelet Rich plasma (PRP) is more effective than triamcinolone acetonide (10 mg/ml) in treatment of OPV at end of treatment period and follow up. Finally, both modalities are believed to be promising in treatment of oral pemphigus vulgaris, so the present work has shown high cure rate with short duration of treatment and no side effects for both modalities.
