Abstract Our study was initiated to determine the role of STE performed at rest to predict the presence of CAD in patients suspected of having CAD. Also to determine the role of STE to predict extent of CAD; and its role to predict the number of affected coronary arteries in patients proven to have CAD by coronary angiography. This was based on previous studies showing that resting tissue Doppler echocardiography helped in the diagnosis & grading of CAD. from the 417 patients, 298 patients (71.5%) were excluded and the remaining 119 patients (28.5%) were enrolled in our study and evaluated by speckle tracking echocardiography & coronary angiography where they were classified into two groups depending on the coronary angiographic finding group I (study group): 50 patients having obstructive (≥50 %) CAD. group II (control group): 69 patients having non obstructive (<50%) CAD Regarding the population studied, the mean age was 50 years with the same demographic data between the two groups. We did full study of speckle tracking echocardiography to assess the global longitudinal strain (GLS) and (lowest longitudinal strain (LLS) from 18 segment module. Results showed that there was generally a statistically significant difference in GLS and LLS between group I and group II. Global longitudinal strain (GLS) showed high sensitivity for the diagnosis of CAD (sensitivity 80%, specificity 90%) with cut-off value (-15.5±3.64) for single vessel CAD; (-11.8±1.99) for two vessels disease and (-5.7±0.86) for three vessels CAD). The lowest longitudinal strain (LLS) showed good sensitivity 78% and specificity 80% for detection of the number of diseased vessels with cut-off value (-6.78±5.22) for single vessel CAD; (-4.8±2.18) for two vessels disease and (-2.5±0.38) for three vessels CAD. Therefore, speckle tracking echocardiography provides an adequate tool for early evaluation of patients suspected of having CAD, providing a safe inexpensive tool & adding some value to the pre-test probability. |
