Abstract

Relapse addiction takes a heavy toll in terms of the social, medical and emotional health of individuals and communities. It might be the primary health issue facing the world in the 21st century. It threatens and affects the overall welfare and health care system (Gordon & de Jong, 2018).
Physical, psychological, social, and economic effects, lost productivity, the cost of treatment, the burden on family and community resources, crime, and psychological distresses common consequences of relapse among addicted persons with persistent relapsing episodes (Melemis, 2017).
In recent years, efforts to address this unfortunate reality are becoming an integral part of treatment and a considerable focus has been widely placed on coping strategies with psychosocial and environmental triggers of relapse that help the addicted persons to identify common high risk factors associated with relapse addiction after achieving partial recovery, avoid and cope with relapse inducing situations and management of these factors (Dobe & Zernikow, 2019).
Aim of the study:
The aim of the study is to assess predisposing factors related to relapse among addicted persons through:
- Assessing the Psychological, Social, Family and Environmental factors related to relapse among addicted persons.
- Assessing of addicted persons’ knowledge regarding addiction and relapse.
- Assessing of addicted persons ’health needs and problems.
Research questions:
Q1: What is the predisposing factors influencing relapse?
Q2: Is their relation between socio-demographic factor and relapse?
Q3: Is their relation between addicted persons’ knowledge about addiction and relapse?
Q4: Is their relation between addicted persons’ needs and relapse?
Research setting:
The study was conducted at the addiction outpatient clinics at El-Abbassia Mental Hospital, and in the Addiction outpatient Clinics, Institute of Psychiatry Ain Shams University.
Sampling:
A purposive sample of 260 addicted persons were selected. The total number of addicted person in El-Abbassia Mental Hospital during the years of 2014 and 2015 was1038 addicted persons and the relapse rate for substances addiction is 10 to 30 percent from the flow of addicted person. 25% of them were taken with the following inclusive criteria:-
- Are range: 18-59 years.
- Free from neurological or psychotic disorder (through checking the patient chart).
- The person has a previous history of substance abuse and at least once of relapse.
Tool of data collection:
● A Structured interviewing questionnaire sheet:
This tool was designed by the investigator and written in simple Arabic language based on scientific literature review to gather data in relation to the following five parts;
Part I: The socio demographic characteristics of addicted persons which include sex, age, marital status, having children, level of education, occupation and monthly income (Q1-Q7).
Part II: Previous history about addiction and relapse: It was divided into:-
(A) History of addiction for addicted person including duration of smoking and addiction, types of substance, beginning of treatment, number of hospitalizations, causes of not compliance to treatment and family history of addicted person and degree of relativeness (Q8-Q20).
(B) Previous history of relapse addiction including the types of substance abuse during relapse, dose, number of relapses, duration of abstinence treatment and family role after relapse and during treatment (Q21-Q26).
Part III: Addicted person’s knowledge regarding addiction and relapse: It included two parts:-
(A) Assessment of addicted persons ’knowledge regarding to addiction including the concept of addiction, factors which leading to addiction, withdrawal symptoms of addiction, sign and symptoms, complication of addiction and stags of developing addiction (Q27-Q32).
(B) Assessment of addicted persons ’knowledge regarding to relapse addiction including the concept of relapse, causes, signs and symptoms of relapse, complication and stages of relapse (Q33-Q37). .
Part IV: Health needs for addicted person related to relapse: It included two parts:-
(A) Assessment of addicted persons’ health needs, which included:
- Physical needs (Q38-Q53).
- Psychological needs (Q54-Q61).
- Social needs (Q62-Q71).
- Financial needs (Q72-Q77).
(B) Assessment of addicted person’s health problems.
- The psychological problems (Q78).
- The physical problems (Q79-Q84).
Part V: Assessment of predisposing factors related to relapse It includes four main variables such as environmental factors, social factors, psychological factors and family factors (Q85-Q88).
The finding of the current study can be concluded that as the following:
More than two fifth of the studied addicted persons had high predisposing factors related to environmental, social, psychological and family factors of relapse .
Highly Statistically significant differences between the studied addicted person’s marital status and their frequencies of relapse with p-value < 0.001,also there was a statistical significant differences between the studied addicted person’s age and their frequencies of relapse with p-value <0.05.Also, there was a highly statistical significant differences between the studied addicted person’s occupation and their frequencies of relapse with a P- value <0.001, also there was a highly statistical significant differences between the studied addicted person’s monthly income and their frequencies of relapse with a p-value <0.001). And there was a statistical significant differences between the studied addicted person’s level of education and their frequencies of relapse with a P-value <0.05, also there was a statistical significant differences between the studied addicted person’s having a children and their frequences of relapse with a P- value <0.05.
Highly statistically significant differences between the studied addicted person’s total knoledge about addiction and their frequencies of relapse with a p-value <0.001.
Highly statistically significant differences between the studied addicted person’s total needs and their frequencies of relapse with a p-value <0.001.
The findings of the present study suggest the following Recommendations:
1- Counsling program for the addicted persons and their families about predisposing factors, causes and consequences of relapse among addicted persons..
2- Discharge plan for addicted persons including follow up visits schedule, the required examinations, and referral numbers for each type of the expected complaints from addiction.
3- Health education sessions for addicted persons regarding the healthy life style programs including healthy nutrition and physical exercising.
4- Further studies should be conducted focusing on studying the health needs and problems for addicted persons.
