Abstract

Background: Multiple promising researches were done regarding using platelet rich plasma (PRP) injection aiming at establishing new minimally invasive treatment option for knee OA and avoiding surgical procedures hazards which could be an option in advanced cases. Thus, our study will be directed towards ultrasound guided injecting of PRP trying to confirm the efficacy of the procedure as a therapeutic option.
Objective: The study aims to assess of the role of ultrasound guided PRP injection in cases of knee osteoarthritis and follow up of the symptoms outcome.
Methods and material: Twenty patients with different knee osteoarthritis grades were injected with PRP under ultrasound guidance and the symptoms outcome were followed up by using visual analogue scale (VAS), WOMAC osteoarthritis index, OXFORD knee score, LYSHOLM knee scoring scale at periods of 3, 6 and12 weeks post procedure.
Results: A total of 20 patients were included with the age ranging from 39-62 years old, the mean was 50.75 years old (SD ±6.16). Among them 16 patients (80%) were females and 4 patients (20%) were males. There was statistically significant difference between the different knee pain scoring scales 3 ,6 and 12 weeks after the procedure compared to before injection, where according to VAS score the mean score was 7.55±1.54 pre procedure and became 6.80±1.61 , 5.90±1.62 and 5.00±1.75 after 3 , 6 and 12 weeks respectively with mean reduction ratio of 25.5 % after 3 months. The extent of the difference over the periods through WOMAC in knee osteoarthritis patients was 50.30±17.46, 48.05±17.77 and 45.25±18.07 after 3, 6 and 12 weeks respectively, compared to 55.70±17.04 before the procedure with a mean reduction ratio of 10.45 % after 3 months. While according to OXFORD knee score, the patient’s scores were 31.40±9.01 before the procedure Vs 33.75±8.61, 35.35±7.86 and 37.35±7.65 at 3, 6 and 12 weeks after injection with symptoms mean reduction ratio of 5.95 %. According to LYSHOLM knee scoring scale, the mean score was 54.35±15.56 pre procedure and became 60.30±17.24, 66.00±17.70 and 69.75±19.35 after 3, 6 and 12 weeks respectively with 3 months mean reduction ratio of 15.4 %.
Conclusion: PRP injection in symptomatic knee osteoarthritis patients cause statistically significant improvement of the symptoms.
