Abstract

Adolescent idiopathic scoliosis (AIS) is a common condition that affects the aolsecent girls and boys responsible for approximately 80 % of all coronal plane spinal deformities A selective thoracic fusion was defined as a fusion in which only the thoracic curve(s) was instrumented and distal fixation terminated at or above the first lumbar vertebra (L1). selective thoracic fusion is now currently widely used for thesurgical treatment of Lenke Type I adolescent idiopathicscoliosis (AIS). The surgical treatment of adolescent idiopathic scoliosis(AIS) aims to achieve coronal deformity correction,sagittal balance, and axial derotation while minimizingthe number of motion segments fused Associated change in the lumbar angle is the main associated effect of surgical correction of AIS by selective thoracic fusion. The aim of this study was to evaluate the effectiveness of selective thoracic fusion on compensatory lumbar curve for adolescent idiopathic scoliosis. The study included 25 patients with adolescent idiopathic scoliosis admitted at Mansoura University Hospital and operated by STF with at least 2 years follow up.All the cases were Lenke type 1 spinal curves. The cases were subjected to full history taking and full clinical examnation. Radiological analysis (Plain X-rays PA, Lateral and side bending views of the spine) in addition to full preoperative laboratory workup. The surgical approach included posterior selective thoracic fusion approach. The cases were regularly followed up after the surgery with Xray radiographs (PA and lateral views). The results of this study showed that : 1. The mean age of the cases was 15.5 ± 2 yaers with range between 10 and 18 years. 2. Twenty three were females (92%) and two cases were males (8 %). 3. No major detected intraoperative complications among the cases included in the study. 4. There was a statistically significant improvement of the major Cobb angle of the thoracic curve measurement postoperatively (68.4% -88.9%). 5. There was a statistically significant improvement of the major Cobb angle of the lumbar curve measurement postoperatively (70.4%-73.3%). 6. Significant correction of the cases with hypo or hyper thoracic kyphosis postoperatively. 7. No significant change in the angle of the lumbar lordosis postopeatively. 8. There were 2 cases with superficial infection (8%), 1 case with rib hump (4%) and 3 cases with distal adding on. Conclusion : from these results we can conclude that : x Adolescent idiopathic scoliosis (AIS) is a common condition of young adults that mostly affects females more than males. x Posterior selective thoracic fusion was assciated with significant improvement of the main thoracic and lumbar Cobb angles of kyphosis. x Decrease of thoracic kyphosis and lumbar lordosis can be encountered with better rotational correction of thoracic spinal deformity. x No major complications associated with posterior selective thoracic fusion especially on the lumbar lordosis.
