Abstract

Meniscal injuries are the second most common injury to the knee with an incidence ranging from 12 to 14%. These injuries can be divided into two groups: traumatic tears and degenerative tears. Degenerative changes are frequently observed in older patients, when articular cartilage is exposed to compression forces resulting in lubricating function impairment of the synovial fluid.
This study aimed to compare the efficacy of arthroscopic debridement of degenerative meniscal tears versus intra-articular hyaluronic acid injection. It was done at El Hadra University Hospital as a Prospective randomized clinical study.
The study included Forty Patients with mild to moderate OA and degenerative meniscal tear, divided into two groups. group (A) was injected intra-articular with hyaluronic acid 60mg in divided doses on three sessions with one week interval, while group (B) underwent arthroscopic partial meniscectomy and debridement.
Patient’s age in group (A) ranged between 40 – 58 years with a mean of 49.80±6.075 years, while in group (B) it ranged between 41 – 60 years with a mean value of 48.85±5.806 years. When talking about the affected side, 12 (60%) were right in group (A) and 7 (35%) were right in group (B).
As regards patient’s gender, group (A) showed that 6 (30%) were males and 14 (70%) were females while in group (B) 8 (40%) were male and 12(60%) were female. Considering the duration of symptoms, those who complained for less than 6 months were 8(40%) in group (A) and 6(30%) in group (B).
Regarding patient’s occupation, group (A) showed that 7 (35%) were employees, 8 (40%) were housewives, 1(5%) was a nurses and 4(20%) were workers, while in group (B) 6 (30%) were employees, 7 (35%) were housewives, 3 (15%) were nurses and 4 (20%) were workers.
The patient’s Kellgran and Lawrence grading scale in group (A) showed that 9 (45%) were mild and 11(55%) were moderate while in group (B) 7 (35%) were mild and 13 (65%) were moderate.
Patient’s smoking in group (A) showed that 15 (75%) were non-smokers and 5 (25%) were smokers, while in group (B) 16 (80%) were non-smokers and 4 (20%) were smokers.
