Abstract

The purpose of the present in vitro study was to evaluate the antibacterial efficacy of photodynamic therapy and high power diode laser against endodontics root canal bacterial biofilms (Enterococcus faecalis or staphylococcus aureus), as compared to conventional cholohexidine irrigation. One hundred and twenty extracted single rooted premolar were used in the study. All teeth were prepared, sterilized, and then divided into two main groups according to the type of bacteria. group (1), the teeth were infected by Enterococcus faecalis biofilms, (ATCC 29212). group (2), the teeth were infected by staphylococcus aureus biofilms (ATCC 29213). Each group was divided into equal five subgroups, according to the type of treatment as follow, Subgroup A [2٪ chlorohexidine]: The infected teeth were irrigated by 2ml of 2٪ chlorohexidine for 2 minute. Subgroup B [photodynamic therapy]: treated by PDT using methylene blue (50ug/ml) and 650 nm diode laser for 2 minutes, Subgroup C [combination of 2٪ chlorohexidine and photodynamic therapy]: The teeth were irrigated by 2ml of 2٪ chlorohexidine for 2 minute then treated by PDT for 2 minute. Subgroup D [high power diode laser]: The teeth were irradiated by (810 nm, 1 watt) diode laser for 2 minutes. Subgroup E [combination of 2٪ chlorohexidine and high power diode laser]: The infected teeth were irrigated by 2ml of 2٪ chlorohexidine for 2 minute then irradiated by 810 nm diode laser for 2 minute. The antibacterial efficacy was assessed by two methods, the colony forming units (CFUs) and confocal laser scanning microscope (CLSM) before and after different type of treatment. The results of the study mentioned as follow, - In subgroup A [ 2٪ chlorohexidine]: 2٪ chlorohexidine solution eradicate Enterococcus faecalis by percentage of reduction was 67,23٪ in (CFUs) and 45٪ in CLSM analysis. While eradicate staphylococcus aureus was 72.89٪ in (CFUs) and 57٪ in CLSM analysis. - In Subgroup B [photodynamic therapy (PDT)]: the photodynamic therapy (PDT) eradicate Enterococcus faecalis by percentage of reduction was 69,32 ٪ in (CFUs) and 58٪ in CLSM analysis. While eradicate staphylococcus aureus was 78.95٪ in (CFUs) and 58٪ in CLSM analysis. - In Subgroup C [combination of 2٪ CHX and PDT]: the combination of 2٪ CHX and PDT eradicate Enterococcus faecalis by percentage of reduction was 92.02٪ by (CFUs) counting and was 63٪ by CLSM analysis. While eradicate staphylococcus aureus was 92.05٪ by (CFUs) counting and was 61٪ by CLSM analysis. - In Subgroup D [high power diode laser]: the high power diode laser eradicate Enterococcus faecalis by percentage of reduction was 96.45٪ by (CFUs) counting and was 77٪ by CLSM. While eradicate staphylococcus aureus was 93.12٪ by (CFUs) counting and was 63٪ by CLSM. - In subgroup E [combination of 2٪ CHX and high power diode laser]: the combination of 2٪ CHX and high power diode laser eradicate Enterococcus faecalis by percentage of reduction was 96.89٪ by (CFUs) counting and was 80٪ by CLSM analysis. While eradicate staphylococcus aureus was 93.12٪ by (CFUs) counting and was 71 ٪ by CLSM analysis. - So the combination of 2٪ chlorhexidine and high power diode laser with 1 watt for 2 minute seems to be the best effective treatment against E.feacalis with reduction percentage 96.89٪ and 93.82٪ for staphylococcus aureus. - The combination of 2٪ CHX irrigation with photodynamic therapy using 650nm diode laser with 100mw output power for 2 minute effective in reduction of E.feacalis by 92.02٪ and the percentage of reduction for staphylococcus aureus 92.05٪. - High power diode laser or photodynamic therapy is a useful adjunct with conventional root canal irrigation with chlorhexidine in disinfection of infected root canals with E.feacalis or staphylococcus aureus.
