Abstract

Summary
Communication is important in transferring patient information between nurses at the change of shift because patient responsibility and accountability is transferred during a shift change. Handing over patient care at the end of shift is an intricate part of nursing care, which requires nurses to use effective communication to transfer patient information from an off-going nurse to the on-coming nurse. The quality of nursing care a patient receives during a shift, to an extent, is usually dependent on the effectiveness of communication between nurses. The information transferred should include the care the patient received from providers during the previous shift. It also includes information about the care provided to the patient before that particular shift, the effect of care, and the plan of care moving forward. The essence is to promote patient safety, and support quality nurse practice. The communication between the nurse completing a shift and the one starting a shift is called hand-off, hand-over, end of shift report, report, and shift report. However for the purpose of consistency, shift report handover used in this paper.
This study aims to assess the knowledge and performance of nurses’ related to shift report handover then, developing and implementing an in-service training program for improving knowledge and performance of nurses’ related to shift report handover. A quasi- experimental research design was used to carry out the study, which was carried out at Cardiac and Digestive System Center at Sohag City which affiliated to Ministry of Health. In all inpatient departments and intensive care units. The study subjects included (45) nurses.
The tools used for data collection included a knowledge questionnaire and observational checklist to observe actual performance of nurses’ regarding shift report handover. A pilot study was done on sample of nurses representing approximately 10% of the main study sample. The actual data collection of the study was carried out from April 2019 to August 2019.
The main study findings were as follows:
The majority of the nurses’ have age less than twenty five years, females, less than five years of experience, single and a Technical Nursing Institute Diploma.
The results revealed poor knowledge test scores of the nurses’ before implementation of the program and significant improvement immediately and after three months of the program implementation. There are high statistical significant differences between nurses’ knowledge at pre, post and follow-up phases as regard to communication process.
The study showed that the program was effective in improving nurses’ knowledge as reflected in immediately and after three months implementation of the program regarding shift report handover.
After the program implementation there was statistical significant differences in all items of knowledge related to shift report handover.
Before implementation of the program the performance of nurses’ of exchange of shift report handover was very low. While there were statistical significant improvement immediately and after three months of the program implementation.
Concerning correlation between nurses’ knowledge and their performance regarding shift report handover. The study revealed that, there were significant positive correlation in pretest of training program phase (r = 0.665 & p= 0.000*), and no significant correlation in post and follow-up phases after implementation of the program.
Conclusion
The nurses’ in the study setting have poor knowledge and low performance of communication process and shift report handover before implementation of the training program, and after implementation of the training program there was effective in improving nurses’ knowledge and performance. There were significant positive correlation between knowledge score and performance score in pre training program. Meanwhile no significant correlation between knowledge score and performance score in post and follow-up phases of training program.
from the study findings, the following recommendations are proposed:
Hospital should using a Health information system to facilitate and help nurses’ to document patients’ data.The applicability of shift report handover should be supported from the hospitals administration. Developing and implementing continuing education programs for nurses’ that will increase their professionalism in documentation. Develop a nursing committee for setting standards of documentation through constructing policies, measuring tools, corrective actions, and follow up activities to ensure the best quality of patient care.
