Abstract

Heart failure (HF) is a major public health problem affecting an estimated 37.7 million people worldwide, with frequent hospitalizations, impaired quality of life, and shortened life expectancy with 5-year mortality about 50%. (Samsky et al., 2013, Al-Omary et al., 2018) Heart failure (HF) is the most rapidly growing cardiovascular condition globally. The estimates of HF prevalence in general, range from 1–2% of the adult population. The prevalence increases sharply from 1% in 40-year-old individuals to 10% above the age of 75 years and doubles for each decade of life. The mean age of patients admitted with a primary diagnosis of HF ranges from 70 to 75 years. (Sidney et al., 2013) Orthostatic hypotension (OH) is a frequent concomitant disorder and a common co-morbidity in heart failure (HF). It has been associated with increased risk of mortality and CV morbidity. Prevalent OH may precede the incidence of heart failure (HF) and atrial fibrillation (AF). (Agarwal et al., 2013, Ricci et al., 2015, Gorelik et al., 2016) The presence of OH is associated with 30% higher risk of CHF incidence in the future compared with those without OH at baseline, especially for the middle-age subjects and those with hypertension and DM. (Xin et al., 2013) There is a close relationship between SBP changes and CVD morbidity and mortality in patients with HF, SBP < 90mmHg and increased long-term BP variability is associated with poor survival in HF. (Ventura et al., 2017) The aim of the current study was to assess postural hypotension in chronic heart failure patients and its association with clinical manifestations and echocardiographic parameters. The study is a prospective cross-sectional observational study that included 100 patients who were presented to National Heart Institute with symptoms and signs of chronic heart failure. All patients with chronic heart failure were subjected to thorough history taking, full examination for symptoms and signs of heart failure, 12 leads ECG, transthoracic echocardiography and 6MWT evaluation. Patients of the study were classified to two groups; group A with orthostatic hypotension and group B without orthostatic hypotension.
