Abstract

Acute lower abdominal pain causes can be grouped under a spectrum of surgical, gynecological, urological and other conditions ranging from trivial to life threatening therefore requiring immediate proper diagnosis and accurate surgical intervention to avoid further health risks and death.
Different strategies and criteria have been used to asses these patients including careful history taking, proper physical examination, imaging methods and Laparoscopy.
The recent evolution of laparoscopic surgery and advancement in different laparoscopic diagnostic techniques have provided a new accurate strategy for proper assessment and surgical intervention of abdominal diseases.
The main goal of diagnostic and interventional laparoscopy is to identify proper diagnosis, fasten treatment time, improve treatment outcomes, avoid the possibility of unnecessary laparotomies and decreasing post-operative morbidity.
This retrospective descriptive study was conducted in 50 patients who underwent laparoscopic evaluation and intervention for the management of lower abdominal pain due to different intra-abdominal surgical conditions for the period of one year at both department of surgery in Alexandria main university hospital and El shatby maternity university Hospital.
In our study we had 23 (46%) of patients with acute appendicitis as the most common cause of lower abdominal pain.
In the present study, gynecological diseases were confirmed in 54% of patients. This is in agreement with most of the literature and proves that the acute abdomen in this age range (especially females) is common and often creates diagnostic difficulties, as young women of childbearing age have a wider differential diagnosis for intraperitoneal inflammation.
In the present study, after the laparoscopic intervention, 48/50 (96%) of patients reported that there was no abdominal pain anymore and that it was resolved.
The advantage of laparoscopic intervention both diagnostic and therapeutic are to enhance immediate treatment process, increase better treatment outcomes such as minimal pain, less blood loss, less infection.
