Abstract

Ankle arthritis is a very common problem, and treatment decisions can be a challenge in young patients. Fifty-percent of elderly patients have some form of arthrosis involving the foot or ankle. Post-traumatic arthritis is the most common cause, representing 70% of cases among patients who present with osteoarthritis.
A thorough clinical examination will help isolate the ankle pathology along with radiological investigations. Weight bearing radiographs are essential. The hind foot alignment view is important when the heel is in varus or valgus or when the ankle has coronal plane tilting.
In most cases of post-traumatic ankle arthritis, it is critical to exhaust an extensive course of non-surgical treatments prior to offering surgical options. The basic options include NSAIDs, a cane, orthotics, orthoses, and injections, which include corticosteroids, hyaluronic acid, and platelet-rich plasma.
Some Patients with end-stage ankle arthrosis often experience significant pain and functional limitations.
Ankle fusion provides a painless, plantigrade, and stable foot. Several different methods have been described to achieve the ankle fusion; including external fixators, internal fixation, and intramedullary nailing however, it is a salvage procedure that causes persistent alterations in gait and a potential for deterioration exists due to the risk of developing ipsilateral hind foot arthritis.
There are several operative techniques for ankle arthrodesis including open or arthroscopic approaches. Successful clinical outcome can be achieved following both approaches.
The primary aim of this study was to evaluate the clinical effect of ankle arthrodesis with screw fixation through the transfibular approach. In comparison with the preoperative values, postoperative AOFAS ankle hindfoot score was markedly improved. 95% bone union was achieved in 20 patients, showing that ankle arthrodesis with screw fixation through the transfibular approach provides satisfactory clinical outcomes. This procedure ensures a high bone union rate and strong stability, eliminates ankle pain, corrects the deformity, and significantly improves ankle function.
