Abstract

BACKGROUND: chronic obstructive pulmonary disease (COPD) is a common preventable and treatable disease that is characterized by persistant respiratory symptoms and airflow limitation, Pulmonary function tests are of great significance in the diagnosis and evaluation of COPD, Forced expiration manuver that is used as a part of spirometry can be difficult for patient to perform as it is effort dependent can alter bronchomotor tone, Impulse oscillometry (IOS) is a noninvasive effort independent easily performed technique and considered as complementary to spirometry Aim of the work: We aimed to evaluate impulse oscillometry system as a diagnosis tool of chronic obstructive pulmonary disease (COPD), in a trial to clarify the sensitivity and correlations of IOS parameters with COPD severity METHODS: This study was carried out on 60 patients with COPD, 30healthy volunteers (control group) 15 smokers, 15 nonsmoker. All the subjects were sequentially examined by Impulse oscillometry system to detect respiratory impedance, and then, spirometry was performed in one time, both repeated (15-45) minute after the first dose of inhalation therapy (salbutamol), chest X ray, WBCs count, CRP and ABG was done for all patients RESULTS: Impulse oscillometric, spirometric parameters were significantly higher in COPD patients as compared with control groups, IOS detect early small airway affection in mild to moderate cases on COPD and there is significant correlation between IOS parameters and spirometric parameter WBCs count, CRP were significantly higher in COPD patients as compared with control groups CONCLUSIONS: IOS is more sensitive in detecting small airway affection in mild to moderate COPD and could be used as alternative method to spirometric assessment in severe COPD patient with more accurate grading.
