Abstract

Acute variceal bleeding is the most feared and devastating complication of cirrhosis; it occurs in one third of patients with varices and causes 70% of all upper gastrointestinal bleeding episodes in cirrhotic patients. Obesity and hypertension have been increased dramatically in the past few years. With the presence of these adjuvant factors in Egypt, hepatitis C and obesity leading to NAFLD and hypertension, the prevalence of liver cirrhosis with hypertension has increased. Therefore, the need to prevent variceal bleeding, which is the leading cause of death in cirrhotic patients, has become mandatory.To our knowledge, this is the first study on the efficacy of carvedilol versus endoscopic band ligation for primary prevention of esophageal varicealbleedingincirrhoticpatientswitharterialhypertension.This study was a prospective, open label, parallel, randomized, controlled trial carried out on 306 patients who were enrolled from endoscopy unit in Tropical Medicine and Infectious Diseases Department, Tanta University Hospital, Egypt. The duration of the study was 24 months (recruitment and follow-up). It began in October 2018 and ended October 2020. The included patients were randomized into carvedilol group (153 patients)orendoscopicbandligation(EBL)group(153patients).Inclusioncriteria Aged18years. Cirrhoticpatientswitharterialhypertension. Endoscopicevidenceofmedium/large-sizedesophagealvarices.
