Abstract

Breast cancer is the most common cancer in women worldwide. For early stage breast cancer patients, breast-conserving surgery (BCS) followed by adjuvant whole breast irradiation (WBI) has become the standard of care and proved to be equivalent to mastectomy. Adjuvant radiotherapy decreases the recurrence risk to 50% and reduces the risk of breast cancer death with nearly 20% after breast conserving surgery. Supine position is the standard position during whole breast irradiation. It provides patient comfort and reproducibility for the whole treatment period. However, in some patients with large pendulous breast, supine position may not be suitable as laterally displaced breast over the chest wall may lead to more inclusion of heart and lung in the radiation field and increase dose to inframammary fold. This allows for significant cardiac, pulmonary and skin toxicities. Prone position is considered an alternative to the standard supine position especially in patients with large pendulous breasts when we need to reduce doses to OAR. It allows the breast tissue to fall away from the chest wall, thus reduces heart and lung exposure and toxicities. It may reduce skin toxicities by improving dose homogeneity and eliminating the bolus effect at the inframammary fold. This is a prospective therapeutic experimental study which included 30 female patients with early stage breast cancer who underwent conservative breast surgery and treated at the Clinical Oncology and Nuclear Medicine Department, Mansoura University Hospital from January 2016 to June 2018.
