Abstract

Ovarian cancer is the second most common gynecologic malignancy. However, it has the highest mortality among all gynecologic malignancies.
Magnetic resonance (MR) imaging provides useful information for the characterization of various ovarian masses. MR imaging is a useful modality for differentiating benign and malignant ovarian tumors, and a specific diagnosis can be made for certain pathologic entities. Diffusion-weighted imaging use may improve MR characterization of ovarian lesions.
The aim of the work was to determine whether the addition of DW sequences to a conventional MR imaging protocol can help in the prediction of the nature of suspicious ovarian masses that are detected previously by conventional ultrasound.
The current study was carried out on thirty-five patients with initial undetermined ovarian lesions during the period between December 2019 and December 2020 at Radiology Department of Alexandria main University Hospital and Radiology Department of Ain Shams University Hospitals. All patients are subjected to full history taking, revision of the patient’s laboratory investigations as tumor markers, and MRI examination. The final diagnosis was reached by surgical findings and histopathological examination.
The mean patient age was 41.00±13.37 years. 17 (48.57%) of 35 ovarian masses were benign, and 12 (34.29%) were malignant, and 6 (17.14%) were borderline. Diffusion-weighted MRI findings of the different histopathological types showed restricted diffusion in 83.3% of borderline lesions and 91.7% of malignant lesions while only 17.6% of benign lesions showed restriction diffusion with a statistically significant difference at p-value 0.025*.
Our results concluded that the DWI can help and increase the confidence of MRI in assessment or exclude potential malignancy in ovarian masses.
