Abstract

Summary
chronic suppurative otitis media (CSOM) is a long-standing infection of middle ear cleft, characterized by ear discharge and permanent perforation of tympanic membrane. Incidence of CSOM is higher in poor socioeconomic group, poor nutrition and lack of health education in rural population.
An unrepaired tympanic membrane perforation can significantly affect a patient’s quality of life. Repairs are required regardless of the size of the perforation because without closure, patients might develop hearing loss, chronic otorrhea and/or cholesteatoma.
Various materials such as fascia, perichondrium and cartilage have been used for the reconstruction of the tympanic membrane in middle ear surgeries.
Platelet-rich plasma (PRP) represents an emerging biotechnology in current tissue engineering and cellular therapy. It is a simple and minimally invasive method to obtain a high concentrate of autologous growth factors (GFs) which can be easily and safely placed directly into the lesion site.
In otolaryngology, firstly used autologous platelet-rich plasma in 2009, that was effectively accelerated tympanic membrane perforation healing in rats.
Since it is an autologous preparation, PRP is inherently safe and therefore it is free from transmissible diseases such as HIV, Hepatitis etc. Also, no toxic effect to the inner ear.
In this work we studied the comparison between platelet-rich plasma (PRP) hourglass graft and traditional method using temporalis fascia graft in myringoplasty.
This study was conducted on 40 patients who were classified into two groups each group had 20 patients. Platelet-rich plasma (PRP) was used in the 1st group and temporalis fascia was used in the 2nd group. All patients were done under general anaesthesia.
All patients were performed pure tone audiometery (PTA) before the operation and three months after the operation and they were compared to each other. Follow-up regarding tympanic membrane healing and improvement of hearing and the appearance of any complications between the two methods and recorded them.
The results revealed that there is no difference between the two methods regarding the absolute reduction in values. Post-operative pain and complications were more in temporalis fascia than PRP group. PRP is rapid (prepared in 12 minutes), low cost and it guards aginst infection.
