Abstract

Tuberculosis (TB) is a contagious bacterial disease that is a major public health problem worldwide. In 2014, there were an estimated 9.6 million new TB cases. More than 90% of new TB cases and deaths occur in developing countries.
Egypt has massively engaged in early case detection, provision of adequate chemotherapy, and prevention of transmission to new cases. Screening, diagnosis, notification, and registration of TB cases were implemented all over Egypt according to the National TB Strategy of the National Tuberculosis Control Program (NTP).
The GeneXpert MTB/RIF test principle is to expand nucleic acid and expose MTB region of the rpoB gene. It also reveals mutations leading to rifampin resistance. It is an automated system that processes sputum, expresses DNA, and amplifies it to diagnose TB.
GeneXpert test seems predestined for use with other speciemens rather than sputum such as cerebrospinal fluid or from other sites such as pleural fluid or ascetic fluid, but data are still scarce, so need further studies for evaluation.
In the current study we aimed to find out tuberculosis pattern in Kafr EL-Shiekh Governorate during the period between January 2010 and January 2019 and role of GeneXpert in diagnosis of TB.
Through study period 1189 cases were recorded 82.5% of them were pulmonary TB and extrapulmonary founded in 17.5% cases.
In current study the mean age was 36.42 years and there was male predominance 57.4%, female was 42.6%.
In current study pulmonary TB cases 8.9% had free radiological findings on the other hand 91.1% had positive findings.
In current study Pulmonary TB was unilateral in 68.3% of cases and 31.7% were bilateral.
In current study Tuberculin test was positive in 99.2% of cases and negative in 0.8% of cases. In current study Sputum positive in 74.2% of cases and negative in 25.8% of cases.
In current study 87.2% of cases treatment completed and treatment failure in 12.8%. In current study Mortality among included cases was 3.1% with higher mortality rate in year 2010 followed by 2011 and lower mortality years 2013 then 2019.
In current study Sensitivity and accuracy of gene expert in diagnosing pulmonary TB cases founded to be 98.19%, and extra pulmonary TB cases founded to be 100%.
