Abstract

Background; Heart failure (HF) is a leading cause of morbidity and mortality in Egyptian patients and negatively impacts quality of life (QoL), healthcare costs, and longevity ,reducing HF burden is a major clinical imperative that should be prioritized to reduce premature death, improve quality of life, and lessen individual and economic burdens of associated morbidities, decreased work productivity, and high cost of medical care. Objective The aim of the study is to find the most significant and strong clinical predictors that can be assessed and predict the rehospitalization among those patient and to reduce the rehospitalization and its burden socially and economically and improve the quality of life for those patients .Patients and methods The study is descrimitive prospective study that included 150 patients who were admitted at specialized medical hospital, Mansoura University by symptoms and signs of heart failure within the time period of October 2019 to October 2021The cases were subjected to full history taking including demographic data, history of the current disease and associated chronic disease, Full clinical examination including general and cardiac examination. Laboratory investigations including analysis of serum sodium, potassium calcium, albumin, full blood count, serum creatinine, INR, uric acid platelet count, hemoglobin level and arterial blood gases, also ECG and echocardiography were conducted for all the participants in the study Results and conclusion: Males were at high risk of readmition than women by 22%for males and 6.8% for females, the most common cause of heart failure readmition is dilated cardiomyopathy ,The most common risk factor associated with readmition among heart failure patients was (COPD) disease with p value 0.002 and odds ratio 4.75 the most clinical symptom associated with readmition is dyspnea .grade III ,the diastolic blood pressure is the most strong clinical sign that can product the rehospitalization with p value <0.001,lower limp edema grade IV is the strongest predictor with B value -19.721 ,ejection fraction is a good predictor for rehospitalization with p <0.001 and cut value of 23.33%.hyponatremia and hypocalcaemia are the most reliable investigation for predicting heart failure patient rehospitalization with p value <0.001.
