Abstract

Background: Young active patients with medial knee osteoarthritis combined with varus leg alignment can be treated with high tibial osteotomy to stop the progression of osteoarthritis and avoid or postpone total knee arthroplasty. Opening-wedge osteotomy are the most commonly used osteotomy techniques. The purpose of this study was to compare the clinical and radiologic outcomes and complications between medial opening wedge (HTO) using Puddu plate with or without bone graft.
Objective: To show medial opening wedge high tibial osteotomy using the Puddu plate with and without bone graft in highly active patients with varus malalignment and medial compartment arthritis and to clarify and identify its benefits and drawbacks.
Patients and Methods: The study between 2010 till 2020. Compare between MOWHTO using Puddu plate with or without bone graft.
Results: The result of this study showed faint difference in time to bone healing clinical and radiographic between patients that had bone graft and those without bone graft using MOWHTO and show that without bone graft not increase the risk of complications, such as, delayed bone union, infection, fracture of lateral tibial cortex, lateral plateau fracture, pain, loss of correction and donor site morbidity which are better for bone healing in without bone graft.
Conclusion: Medial opening wedge high tibial osteotomy which is a good and valuable treatment option for young, active patients with knee osteoarthritis has improved good long-term outcomes with appropriate patient selection and careful surgical technique.
