Abstract

Aim: To compare the effects of combined maxillary and mandibular posterior teeth intrusion with maxillary posterior teeth intrusion only on the amount of anterior open bite (AOB) closure.
Methodology: Twenty-two patients were randomized to either the comparator or intervention groups. Levelling and alignment of maxillary and mandibular posterior segments was done. Miniscrews were placed in the infra-zygomatic and palatal areas (between first and second premolars) for maxillary teeth intrusion. Patients in the intervention group received two more miniscrews in the mandibular buccal area between the second premolars and first molars. Closed nickel-titanium coil springs and memory chains were used to deliver the forces for the maxillary and the mandibular teeth respectively. Results: Anterior open bite was significantly closed in both groups with means of 3.8 ± 0.84 and 3.84 ± 1.47 mm for the comparator and intervention groups respectively, with non-significant difference between them. Likewise, maxillary posterior teeth experienced significant intrusion in both groups, with a mean of 2.89 ±1.13 in the comparator group and 2.26 ± 1.62 mm in the intervention group. There was a mean of 0.63 mm between the groups, which was statistically, but not clinically significant. Statistically significant mandibular posterior teeth intrusion occurred in both groups with means of 0.86 ± 0.91 and 0.33 ± 0.84 mm in the intervention and comparator groups respectively. The difference between the groups had a mean of 0.53 mm and was statistically significant.
Conclusions: Comparable anterior open bite closure can be successfully achieved employing either maxillary or bi-maxillary miniscrew-supported posterior segments’ intrusion. Accordingly, there might not be a need for mandibular posterior teeth intrusion, as long as they’re efficiently consolidated during maxillary posterior segments’ intrusion.
