Abstract

Acute kidney injury (AKI) is a frequent organ dysfunction in critically ill patients, and it increases both short and long term mortality.The study included 80 critically ill patients with sepsis, 40 patients developed acute kidney injury and 40 patients without acute kidney injury. All patients were subjected to full battery of history, clinical examination, laboratory investigations, renal resistive index and estimation of serum IL 18 and KIM 1 levels. Aim of the study:Aim of the work is to detect the value of serum level of Kidney injury molecule - 1 (KIM-1) and Interleukin 18 (IL 18) and renal resistivity index as early predictors of acute kidney injury in critically ill patients with sepsis.Also we aim to determine the role of Kidney injury molecule- 1 (KIM-1), Interleukin 18 (IL 18) and renal resistivity index in predicting 30 days mortality in ICU patient. We found that:ROC analysis for detection of acute kidney injury 1 day after admission showed that AUC for serum IL 18 is 86.1%, AUC for the serum KIM-1 is 86 % and AUC for RI is 88 %, demonstrating a statistically significant good performance of these markers for the diagnosis of AKI within the next 24 h. Highest sensitivity was found for IL-18 while highest specificity was found for KIM-1.Also we found that ROC analysis of serum IL 18, serum KIM 1 and RI for prediction of 30 days mortality showed that all demonstrated a statistically significant good performance for detection of 30 days mortality with the highest AUC and sensitivity were for RI while the highest specificity was for IL 18
