Abstract

Background & objectives: Pregnancy registries for women with epilepsy (WWE) are arising all over the world. The aim of this work was to assess seizure control and determinants of outcome in pregnant WWE through the Egyptian Registry of Antiepileptic Drugs and Pregnancy system. Methods: An observational prospective study was conducted over 24 months (2018-2020). The following data were assessed: seizure control 6 months before pregnancy, antiepileptic drugs (AEDs) regimen, folic acid supplementation, frequency of seizures and birth outcome. Results: This study included 211 pregnant WWE, with mean age of 27.30 ± 5.51 years. Sixty-nine (33.5 %) patients received appropriate folic acid, while 167 (80.7%) patients received folic acid during 1st trimester. One hundred eighty-six (89.9%) patients were on AEDs, from which 110 (59.1%) patients were on monotherapy. One hundred sixty-nine (80.0 %) had healthy living babies, while intrauterine deaths (IUDs) occurred in 27 patients (12.8%), while 13 patients (6.2%) had living babies with major congenital malformations (MCMs). Seventy-three (58.4%), were seizure-free during their pregnancy. Uncontrolled seizures 6 months before pregnancy, would increase the likelihood of breakthrough seizures during pregnancy by 13.79 times (OR= 13.79) (95% CI 5.7{u2013}32.9), and the later would increase the likelihood of seizures occurrence during postpartum-period by 13.055 times (OR= 13.055) (95% CI 4.823{u2013}35.337). Taking folic acid in the first trimester and being seizure-free during pregnancy were protective factors against IUDs (RR<1). Polytherapy exposure increased the risk of MCMs (RR>1)
