Abstract

Aim: To study urological complications after surgical correction of female patients with congenital adrenal hyperplasia (CAH) with partial urogenital sinus mobilization. Materials and methods: Our prospective study included 18 CAH female patients who presented to Diabetes, Endocrinology and Metabolism Pediatric Unit (DEMPU) of Cairo University Specialized Pediatric Hospitals (CUSPH) in the period between august 2018 to august 2020. All cases were diagnosed and managed by DSD endocrinology and urology team. We chose patients who were not previously operated with Prader grade of virilization III-V who were toilet trained. Preoperative assessment of urological condition by Urine analysis, urine culture, International Consultation on Incontinence Questionnaire (ICIQ), frequency volume charts, abdomino pelvic ultrasound and ultrasound for residual volume were done. A single team of surgeons performed single stage genital reconstruction using partial urogenital sinus mobilization.Postoperative assessment by the same tools together with uroflowmetry and examination under anaesthesia (EUA) after one and half months were done to assess postoperative urological and cosmetic outcome. Follow up of our patients extended from 12 to 24 months (mean 19 ± 3.956 SD). Statistics was performed using SPSS version 21. Data were expressed as means ± standard deviation. Correlations were done using Ficher’s exact test. Results: Age at time of surgery ranged from 2.1 to 15 years (mean age: 5.47 ± 3.6 SD). All patients had 21- hydroxylase enzyme deficiency. Positive consanguinity was found in 13 patients (72.2 %). Eight patients were classified as Prader type 3 (44.4 %) and 10 patients as Prader type 4 (55.6 %). All patients underwent genitoplasty using partial urogenital sinus mobilization (100%).Cosmetic appearance was assessed after 1 month, wound healing was found good in 14 patients (77.8%), satisfactory in 3 patients (16.7%) and poor in 1 patient (5.6%) due to poor endocrinal control
