Abstract

O
ur study included 73 patients who divided into two groups, group (A) pneumatic 37 patients and group (B) laser 36 patients, all having multiple stone, or stone size 2 cm or more.
The study was designed to evaluate perioperative outcomes between holmium Yag laser with pneumatic lithoclast in management of patients with renal stones (>2 cm) undergoing percutaneous nephrolithotomy (PCNL).
The study demonstrated a statistically significant difference in operative time, need for intra-operative DJ application and post-operative need for further intervention between both groups, with shorter operative time in the pneumatic lithotripsy compared to laser lithotripsy.
The study has also demonstrated that there is no statistically significant difference between the two groups as regarding to stone free rates, post operative complication or hospital stay.
Conclusion:
In our experience we found that Holmium: YAG laser and pneumatic lithotripsy were both effective lithotripters during PCNL with comparable stone free rates. Laser lithotripsy had longer operative time, but less perioperative complications and less need for DJ application.
