Abstract

Introduction: Diabetic nephropathy is a serious microvascular complication implicated in the death of diabetic patients, which would necessitate the need for new biomarkers to detect early stage of diabetic nephropathy (DN). Neutrophil Gelatinase Associated Lipocalin (NGAL); a glycoprotein present in the kidneys at the tubular cell level which is used in assessing tubular lesions in diabetic patients. It shows increased values even in the initial phases of the disease, especially in normoalbuminuric patients. Objective: To estimate and evaluate NGAL as a biomarker for DN. Patients and methods: This cross-sectional study was carried on 60 male and female type I diabetic children, classified as 30 normoalbuminuric diabetic children (Urinary albumin creatinine ratio <30mg/dl) and 30 microalbuminuric diabetic children (Urinary albumin creatinine ratio 30-300 mg/dl). 30 age matched control children also were included. Detailed history and physical examination, laboratory investigations were done. Urinary NGAL level was assessed in all patients. Results: A very highly statistically significant difference in urinary NGAL between normoalbuminuric diabetic ,microalbuminuric diabetic children and healthy control group with P value<.001. Conclusion: Tubular injury markers such as NGAL can be used as an early marker for DN
