Abstract

Background Juvenile Systemic lupus erythematosus (JSLE) is a chronic autoimmune inflammatory disease characterized by autoantibodies directed against selfantigens, immune complex formation, and immune dysregulation, resulting in damage to essentially any organ. Both hypothyroidism and hyperthyroidism have been found in JSLE patients.Aim To evaluate the frequency of autoimmune thyroid dysfunctions in juvenile SLE and its relation to disease activity and duration. Methods This is a cross sectional study involved 50 JSLE patients who were attending regularly to follow up at rheumatology clinic of Cairo University Specialized Paediatric Hospital. Cases were assessed by complete history taking, a detailed physical examination was performed with a particular emphasis on goiter (if present). All patients were subjected to measurement of serum TSH, Free T3, Free T4, anti-thyroglobulin antibodies (anti-TG) and thyroid peroxidase (anti-TPO) by ELISA. The SLE disease activity score (SLEDAI) was used to evaluate the disease activity. Results 32 out of 50 patients (64%) demonstrated thyroid dysfunctions, 19 patients (38%) with euthyroid sick syndrome, 10 patients (20%) with hypothyroidism, 3 patients (6%) with subclinical hyperthyroidism and none of the patients had hyperthyroidism
