Abstract

The aim of this study was to evaluate the value of uterine (UtA), umbilical (UA), middle cerebral (MCA) arteries and ductus venosus (DV) Doppler indices and waveform patterns in prediction of intrauterine growth restriction (IUGR) and preeclampsia in hypertensive disorders with pregnancy.Early screening of IUGR and preeclampsia may allow proper antenatal surveillance and proper timing of fetal delivery. A sample of 62 pregnant hypertensive and 64 non hypertensive groups were assessed at any period started from the mid/end of the 2nd trimester (around 20-24 weeks of gestation) till the end of the 3rd trimester, included in our study with selection criteria to exclude congenital fetal anomalies, twin pregnancies and pregnant mothers with any other medical disorder.There were significant statistical differences in the Doppler indices of the uterine, umbilical, middle cerebral arteries between the two study groups. By follow up, there was an association between those abnormal Doppler indices of previously mentioned arteries and the incidence of preeclampsia and/or IUGR development. So, this study concluded that:Combined notch with high Doppler indices of uterine artery has a role in prediction of preeclampsia in hypertensive disorders with pregnancy.The Cerebroplacental ratio (C/P ratio = MCA PI / UA PI) is a better predictor of IUGR and adverse fetal outcome than umbilical or middle cerebral alone. Ductus venosus Doppler may have a role in proper management of fetuses with abnormal umbilical artery waveform or abnormal MCA indices
