Abstract

Background: The increasing ratio of elderly population has contributed to increased incidence of TBI in this cohort.this study addresses the clinical characteristics, management strategy, and outcome in elderly patients with traumatic brain injury (TBI) Objective: This thesis aims to evaluate outcome of traumatic brain injury in elderly patients. Methods: Cross sectional analytic study with random sample of 132 patients, which was designed to assess clinical characteristics, management strategy, and outcome in elderly patients with traumatic brain injury (TBI) admitted in Kasr Al Ainy hospital. Assessment of the trauma survey before admission, surgery, Glasgow coma scale on admission and on discharge , mortality and Glasgow outcome score was done. Results: out of 132 patients ,110 patients (83.3%) were males ,22 patients (16.7%) were females , the mean age was 68 ± 7, 66 patients (50%) had history of repeated minimal trauma , 52 patients (39.4%) had history of disturbance of conscious level , 100 patients were mild trauma (GCS 13-15), 67 patients (50.8%) had chronic subdural hematoma , 52 patients (39.4 %) were known hypertensive , 84 patients (63.6%) were on medications , 72 patients (54.5%) had operations , in the follow up only 1 patient (0.9%) had resurgery, in this study 16 patient (12.1%) died Conclusion: Elderly patients who have sustained isolated severe TBI may present with a higher GCS than younger patients due to cerebral atrophy. elderly patients presented with TBI even if CT brain was free should be admitted under observation at least 24. severe Head injury and contusions, Sub arachnoid hemorrhage, intra ventricular hemorrhage are poor prognostic factor sin elderly patients. control of comorbidities and follow up of the co-morbidities help to improve the general outcome of the elderly populations after TBI . co-morbidities such hypertension , diabetes ,ischemic heart diseases are poor prognostic factors in elderly patients with TBI so it is very important to follow up these co-morbidities
