Abstract

Legg-Calve-Perthes disease (LCPD) is a childhood condition characterized by femoral head osteonecrosis. The osteonecrotic lesion eventually heals, but persistent deformity to the proximal femur and acetabulum might induce hip discomfort and dysfunction following recovery. Previous reports have noted that between 30% and 50% of individuals affected by LCPD to have residual hip symptoms that persist and a frequent precursor to hip osteoarthritis in young adulthood.
The deformity resulting from LCPD including a highriding greater trochanter, a short femoral neck, and aspherical femoral head causes femoroacetabular impingement (FAI) and/or instability of the hip which may finally cause damage and subsequently osteoarthritis (OA) of the hip resulting in hip pain, restricted range of motion, and impaired abductor function. The morphology of the proximal femur after healed Perthes disease is the single most important determinant factor affecting the long-term outcome.
Since a standard health-related quality of life questionnaire’s responses revealed that patients who received total hip replacement for Perthes’ disease sequelae had a worse quality of life. These findings highlight the essential need for early preventative intervention to preserve the sphericity of the femoral head during the early stages of Perthes’ disease.
In this review we are going to discuss the different modalities for management of Legg-Calve-Perthes disease consequences, in the way of detecting the most reliable, safe method for treatment of intra-articular pathologies of the hip-joint.
