Abstract

Purpose: To evaluate the outcome after closed reduction and pinning using a Kirschner wire inserted vertically through the olecranon and another inserted laterally for displaced supracondylar humeral fractures in children in the hands of junior trainees. Methods: 22 boys and 18 girls aged 2 to 12 (mean, 5.57) years underwent closed reduction and pinning using transolecranon + lateral Kirschner wires. Transolecranon wire directed posteromedially (n=16), posterolaterally (n=11), posteriorly (n=9), or intramedullary (n=4), in a displaced extension-type supracondylar humeral fractures (Gartland types III). Results: Copared with the uninjured elbows, the injured elbows had a mean±standard deviation (SD) loss of flexion of 2.95{u00BA}+2.32 (p=0.802), a mean±standard deviation (SD) loss of extension of 2.51{u00BA}+ 2.98 (p=0.802), and a mean±standard deviation (SD) change in carrying angle of 9.37{u00BA}+2.52 (p=0.906). According to the Flynn grading system, outcome was excellent in 25 patients, good in 13, fair in 1 and poor in 1. Outcome did not differ significantly between patients aged 2 to 4, 5 to 8, and 9 to 12 years also there are any significant difference between those operated on within 24 hours of injury and those operated after. 2 patient developed superficial pin tract infections and 1 patient have broken wire intraarticularly and No patient sustained iatrogenic nerve injury
