Abstract

Although gonioscopy is the current reference standard for the assessment and diagnosis of angle closure, it is technically challenging, may be uncomfortable for the patient, and subject to inter and intra-observer discrepancies. Imaging of the ocular anterior chamber is gaining increasing importance for the diagnosis, management and follow-up of angle closure disease. The ability to capture the entire anterior chamber in a single image using AS OCT allows better assessment of the structures relative to each other. Our study is a prospective, observational cross-sectional study aiming to evaluate the significant value of AS-OCT parameters in subtypes of primary angle closure and to determine the characteristics of each subgroup. The current study comprised three subgroups of PAC disease; PACS, PAC, PACG. Patients with history of previous ocular surgery, pathologies leading to cloudy media, secondary glaucoma were excluded from the study as this would interfere with imaging by ASOCT. The study involved 60 eyes of patients with primary angle closure patients with 18 eyes of PACS, 18 eyes of PAC and 24 eyes of PACG patients. All the study participants underwent full ophthalmological assessment with gonioscopy using the Goldmann 3 mirror goniolens. Anterior chamber angle parameters were measured using swept source (Topcon Dri Oct Triton, Tokyo-Japan). Quantitative evaluation of the angle has been done using several parameters as anterior chamber width, anterior chamber depth, lens vault, angle opening distance at 500 & 750 μm in front of scleral spur, iris thickness at 750 μm in front of scleral spur, iris curvature, and trabecular iris contact length. In this study, the average mean values of AOD 500 μm, AOD 750 μm, lens vault, iris thickness 750 μm, iris curvature, and TICL showed significant difference in between our study groups. While the average mean values of ACD, ACW showed statistically non-significant differences between our study groups. Our study showed that there was significant negative correlation between lens vault and ACW also TICL at temporal angle in PAC group. Significant negative correlation was found also between lens vault and AC depth in PACG group, while significant positive correlation between lens vault and TICL at temporal angle in PACG group. Stepwise linear regression analysis was performed to identify predictive factors of PACG and it showed that C/D ratio, lens vault, AOD500 at nasal angle, iris curve at nasal and temporal angle, iris thickness at temporal angle, TICL at both temporal and nasal angle were the strongest predictive parameters of subtypes of primary angle closure. Additionally multinomial logistic regression analyses were performed, and showed that AOD 500 at nasal angle, iris thickness at temporal angle, TICL at nasal angle were the strongest predictive parameters of subtypes of primary angle closure.
