Abstract

Indication of previous C.S has its impact on lower uterine segment thickness in the subsequent pregnancyPatients who had C.S for permanent indications (previous uterine surgery,CPD,….)and those for antepartum Hgh(placenta previa,..) had the thinnest L.U.S.T Patients who had C.S for medical disorder(gestational diabetes,SPET,…..) and those for multiple gestation(twins first breech,triplet,…) had L.U.S.T ≥3.3mm. Transvaginal ultrasound has to be used for all women with previous one C.S after 36weeks gestation who wish to have a chance of VBAC. Measuring lower uterine segment thickness is more accurate than measuring myometrial thickness only Regarding the personal characters the mean of maternal age for all patients was ±27 years old . There was no statistically significant difference as regard to gravidy,parity and method of calculation of gestational age. The rate of VBAC between patients of group A (300 patients with previous one C.S) was 29.7% while the rate of C.S for the rest of patients in the same group was 70.3%.For patients who had VBAC (89 patients out of 300 with previous one C.S) the percentage for previous history of VBAC accounts for 61.8% in contrast to 38.2% for those who had VBAC for the first time. According to WHO recommendations for IPI of ≥18ms for patients of group A(300 patients with previous one C.S);patients who had IPI≤ 18 ms had a thin lower uterine segment thickness(2.12 ± 0.84 mm) in contrast to those who had IPI≥ 18ms had a thicker lower uterine segment thickness(3.01 ± 1.05mm). Patients with C.S have a lower uterine segment thickness thinner than those of un scarred uterus.
