Abstract

Introduction: anterolateral ligament (ALL) is a distinct ligamentous which has been found to play an important role in rotational control of the knee. Patients and methods: A RCT was conducted on 40 patients who had ACL tear and grade 3 pivot shift. Each group had 20 patients; group (A) underwent ASB ACL reconstruction and ALL reconstruction while group (B) underwent ASB ACL reconstruction only. Quadrupled semitendinosus was used for ACL reconstruction in all patients and doubled gracilis for ALL reconstruction in group (A) patients. Results: After 12 months follow up, there was no statistically significant difference between the 2 groups regarding Lysholm score, Lachman test and pivot shift test. The mean side to side difference of anterior tibial translation using KT-1000 was in favor of group (A); 0.7mm vs. 1.4mm for group (B), whuch was statistically significant. Mean side to side tibial rotation angle in group (A): -2{u00B0} and while in group (B): -5.1{u00B0} which was statistically significant in favor of group A. Conclusion: There is a statistically significant difference between combined ACL+ ALL reconstruction versus ACL reconstruction only regarding side to side difference of anterior tibial translation measured by KT-1000 and tibial rotation angle in favor of ACL+ALL reconstruction however there is no significant statistical difference regarding Lysholm score, Lachman and pivot shift tests
