Abstract

Metatarsal fractures are Common injuries of the foot frequently seen in emergency departments. The base of the fifth metatarsal is the commonest one.
Classifications of these fractures are based on anatomical region, patient history and radiological findings. Depending on these classifications and patient’s activity level, treatment can be conservative or operative.
There has been rising concern to treat this injury operatively especially in Jones fracture of base of 5th metatarsal prone to complications with conservative management.
The study conducted on adult patients with acute Jones fracture. The study was carried out on 20 patients attending to Orthopedic Surgery Department, Faculty of Medicine, and Beni-Suef University during the period between February 2020 and July 2020 after ethical approval by Faculty of Medicine Beni-Suef University Research Ethical Committee (FM-BSU-REC) from the ethical point of view.
The study include 20 patients, 15 were males (75.0 %) but 5 were females (25.0%). Mean of age was 33.65 with range from 19 to 59 years (Table 1). 11 patients (55%) were right side and 9 patients (45%) were left side. the Type of fixation of 10 patients (50%) was cast and of the other 10 patients was screw.
All patients hadn’t Intraoperative Complications. Only 2 patients (10%) were delayed or non-union, 3 patients (15%) had pain during movement and no patients had infection.
By Comparison between types of fixation among demographic data there was no statistically significant difference between types of fixation. By Comparison between types of fixation among side there was no statistically significant difference between types of fixation.
By Comparison between types of fixation among intraoperative complications shows that study between types of fixation among intraoperative complications was not applicable. By Comparison between types of fixation among delayed or non-union there was no statistically significant difference between types of fixation.
By Comparison between types of fixation among pain during movement there was no statistically significant difference between types of fixation. By Comparison between types of fixation among infection shows that study between types of fixation among infection was not applicable.
We recommend early screw fixation in the treatment of acute Jones fracture in patients with high demand physical activity who want to return early to their work, but also we have to take in consideration the financial cost of the operation versus the cast application. So tailoring of the management plane to the patient activity and financial capability is of relative importance.
Early screw fixation is safe and effective method for treatment of acute Jones fracture of base of 5th metatarsal. Early surgical treatment results in earlier clinical union and allows patients to return to normal daily activities more than the cast treatment which results in high rate of nonunion.
