Abstract

The incidence of varices in cirrhotic patients is about 60-80%. Endoscopic variceal ligation (EVL) has been considered as one of the best emergency as well as prophylactic haemostatic treatments (Jensen, 2002 and de Franchis, 2005). EVL aims to obliterate the varix by inducing ulceration with scar tissue formation (Polski et al., 2001).EVL is considered safe with few treatment-related complications (Tait et al., 1999).However, significant complications occur in about 4% of patients who undergo EVL. The most frequent reported complications include transient dysphagia, post ligature pain and esophageal stricture formations (Saltzman and Arora, 1993; Schmitz et al., 2001 and Garcia-Pagan and Bosch, 2005).In about 3.6-5.7% of patients who undergo EVL, early rebleeding (bleeding occurring between 24 h and 14 d after the operation) can occur from the ulcer base before healing, which is difficult to treat and fatal (mortality up to 52%), and is mainly due to early spontaneous slippage of rubber bands leaving the unhealed ulcer (Li et al., 2006; Petrasch et al., 2010 and Vanbiervliet et al., 2010). Despite the significant associated morbidity and mortality, risk factors for EVL-induced ulcer bleeding have not been clearly defined. Only few studies have reported the possible risk factors forpost-banding ulcer bleeding (Li et al., 2006 and Vanbiervliet et al., 2010).This work was done to determine the frequency and the risk factors of post-banding ulcer bleeding following EVL in patients with liver cirrhosis in order to minimize the post-banding ulcer bleeding. This prospective cohort study was conducted in the Intensive Care Unit (ICU), In-patient and Endoscopy Units of Tropical Medicine Department, Zagazig University Hospitals, during the period from March 2017 to November 2017.
