Abstract

Background: Carpal tunnel syndrome (CTS) is the most common peripheral nerve lesion in the upper limbs. Many patients with CTS submitted to surgical release, functional recovery needs to be accelerated after surgery.
Objectives: To investigate the effect of low-level light therapy (LLLT) on residual pain and morphological changes of the median nerve (MN) after surgical decompression of CTS.
Design: A prospective randomized controlled trial.
Setting: The study was conducted at the outpatient clinic of Banha University Hospital – Egypt, between June 2017 and Feb 2018.
Participants: Thirty patients with residual pain after 3 months from open carpal tunnel release (CTR) were randomly divided into two equal groups.
Interventions: The laser group (A) received 6 weeks treatment plan of 12 sessions of 850 nm gallium-aluminium-arsenide Laser, with a power of 100 mW; in addition to medical treatment (analgesics). The control group (B) received medical treatment (analgesics) alone.
Main Outcomes Measures: The primary outcome was pain measured by visual analogue scale (VAS), secondary outcomes included the morphological changes of the MN; Phalen’s test for assessment of CTS; nerve conduction studies for MN sensory distal latency (SDL) and ultrasonography for MN cross sectional area (CSA).
Results: There was a statistically significant (p<0.05) reduction in VAS score, and improvement of SDL, CSA in laser group compared with control group.
Conclusion: The 850nm gallium-aluminium-arsenide is effective in the management of residual pain and median nerve morphology after CTR.
