Abstract

Background: The treatment of patients with infected tumor prostheses is often challenging and time consuming, with a high number of reoperations required. Early diagnosis, selection of an appropriate treatment strategy, accurate identification of the responsible pathogens, and construction of an appropriate antibiotic regimen are essential elements of any management strategy. Objective: This study was designed to evaluate all the management procedures used for infected tumor prosthesis and the final functional outcome of each treatment modality. Methods: We analyzed the management of each case of the 31 infected cases in details and the final status of each case was evaluated using the musculoskeletal tumor society (MSTS) scoring system. Results: In our study we had a relatively high success rate of conservative management (21.7%) and the mean of the functional score was (83%). The success rate of staged revision was (67%) and the functional outcome of the patients was varying according to the type of the second stage, so in patients with re-implantation of prosthesis as the second stage the mean functional score was (76%) and in patients with fibular graft as the second stage the mean functional score was (63%) and in the patients who remained on the cement spacer the mean functional score was (74%). Conclusion: Inspite of the difficulty of conservative therapy for deep infection and low success rate in literature, yet if the conservative treatment could be successfully completed, patients can benefit greatly being not exposed to repeated revision surgeries and retaining a better functional outcome. Two staged revision surgery has high success rates of clearance of infection with varying degrees of functional outcome depending on the type of second stage and the number of surgeries to which the patient was subjected
