Abstract

Background: In adult living donor liver transplantation (LDLT), a narrow safety margin for the recipient because of graft size disparity and to overcome this in middle hepatic vein (MHV) dominant liver, reconstruction of MHV tributaries is mandatory to prevent liver congestion and dysfunction. Many interpositional vascular grafts are used like cryopreserved and autologous vessels although possible difficulties and time consuming during their preparation. Ringed polytetrafluoroethylene (PTFE) grafts can be used as good alternative in reconstruction. Objective: Our study aimed to evaluate using of ringed PTFE grafts to reconstruct MHV tributaries (of segments V and VIII) to prevent liver graft congestion. Material and Methods: from January 2014 to December 2016, fifteen adult recipients underwent living donor liver transplantation using right lobe grafts without the middle hepatic vein. 12 patients at Dar Alfouad hospital, 6th of October city, and 3 patients at Kasr Elaini hospital Cairo, Egypt. Eight mm ringed PTFE grafts were used to drain 8 segment V vein tributaries, 7 segment VIII vein tributaries. Follow up was done using liver function tests and duplex ultrasound to evaluate the patency of vascular graft and liver congestion. Results: Intra-operative and during period of follow up duplex ultrasound confirmed the patency of the vascular graft and there is no associated congestion of liver with improving of liver functions. Conclusion: ringed PTFE grafts can be used safely and handled easily in reconstruction of middle hepatic vein tributaries to facilitate outflow of liver graft during living donor liver transplantation
