Abstract

Background: Psoriasis is a chronic relapsing inflammatory skin disease that associated with morbidities that extend beyond the skin. It is triggered by an activated cellular immune system involving innate and adaptive mechanisms. Vascular endothelial growth factor (VEGF), a pro-angiogenic factor in the skin, is known to induce pathological angiogenesis in psoriatic patients. Aim of the work: The aim of this study is to to evaluate the antiangiogenic effect of PUVA in psoriatic patients. Patients and Methodology: The study included 30 psoriatic patients, 3 patients dropped during the study. PASI score were done to all patients pre- and at 12 weeks post-treatmemt. VEGF was measured by Enzyme Linked Immunosorbant Assay (ELISA) before initiation of therapy and at the end of the study. Results: Study results showed a statistically significant decrease in the VEGF level in the sera of the patients (p<0.001) and significant decrease in PASI score of the patient (p<0.001) after the treatment period. Conclusion: The present study demonstrated that PUVA therapy exerts antiangiogenic effect. PUVA resulted in excellent improvement of the clinical picture in psoriasis patients. The present study proved that because of its efficacy and its lower cost, PUVA will for a while remain the first choice therapy in the management of psoriasis vulgaris involving more than 30% of the body surface area in patients free from contraindications for this therapy
