Abstract

The aim of the present study was two fold: First to compare the anesthetic efficacy on using bupivacaine alone or with the addition of mannitol for inferior alveolar nerve block (IANB) in patients with symptomatic irreversible pulpitis in mandibular posterior teeth. Second to assess the postoperative pain after single-visit root canal treatment. Subjects and methods sixty patients with symptomatic irreversible pulpitis in mandibular posterior teeth and were medically free were assigned into 2 equal groups. One group received 1.8 ml of 0.5% bupivacaine with 1:200000 epinephrine alone and the other group received 1.8 ml of 0.5% bupivacaine with 1:200000 epinephrine plus 1.5 ml of 20% mannitol. In all cases the root canal treatment was completed in a single visit, the patients were asked to record the degree of pain during access cavity preparation and postoperatively after 6, 12, 24, 48 and 72 hours using Numerical rating scale. The patients were also asked to record lip numbness onset and duration, incidence of taking placebo and analgesics and number of analgesics tablets received. Results The results showed that there was no statistically significant difference between the two groups regarding pain during access cavity preparation and postoperative pain after 6, 12, 24, 48 and 72 hours and there was no statistically significant difference between the two groups regarding lip numbness onset and duration and incidence of taking placebo and analgesics
