Abstract

• Carotid stenosis plays an important role in determination of prognosis of anterior circulation ischaemic stroke.
• We found a relation between carotid stenosis in anterior circulation ischaemic stroke and higher NIHSS.
• We found a relation between carotid stenosis in anterior circulation ischaemic stroke and higher mRS.
• We found a relation between carotid stenosis in anterior circulation ischaemic stroke and higher morbidity and mortality whether stenosis was at the same or at the other side of stroke.
• We found no significant relation between laterality of carotid stenosis and NIHSS (whether unilateral or bilateral).
• We found no significant relation between laterality of carotid stenosis and mRS (whether unilateral or bilateral).
RECOMMENDATIONS
• Strict and proper management of carotid stenosis in acute ischaemic stroke to lessen the burden of after stroke morbidity and mortality.
• Carotid duplex must be done to all patients of ischaemic stroke to determine site, criteria and percentage of carotid stenosis.
• Patients of stroke must be investigated bilaterally by carotid duplex as both bilateral and unilateral symptomatic carotid stenosis at the same side groups had worse prognosis than control group (higher NIHSS and mRS).
SUMMARY
Cerebrovascular stroke is the second cause of death and considered as a major cause of disability worldwide. Ischemic stroke is more frequent than haemorrhagic. Incidence and mortality of stroke differ between countries, geographical regions, and ethnic groups.
Carotid artery stenosis is a major risk factor for ischemic stroke. The cerebral arteries affected due to carotid artery disease are anterior cerebral artery and middle cerebral artery. Symptoms vary according to site of affected blood vessels.
Therefore, investigating prognostic significance of carotid stenosis is important to evaluate its burden on disability and death of patients of stroke and encourages on screening and early detection of carotid stenosis to prevent its effect on socioeconomic level and disability of the patient and economy of the country.
Our study aimed to describe the correlation between presence of carotid stenosis in anterior circulation ischaemic stroke and disability and death.
The patients were selected from inpatients in neurology department at Ain Shams University Hospitals in the period (from July 2021 to August 2022).
The study included Fifty patients more than eighteen years old with acute anterior circulation ischaemic stroke, documented by CT brain within 48 hours of symptoms onset, with no past history of previous disability (25 cases with anterior circulation ischaemic stroke and associated carotid stenosis as patient group and 25 cases with anterior circulation ischaemic stroke without carotid artery disease as control disease), the two groups were matched as regard age, sex and risk factors.
All patients included in the study were subjected to detailed medical history and examination with detailed neurological examination including assessment of NIHSS on admission and mRS one month later.
Also, all patients were subjected to carotid duplex and percentage of carotid stenosis was measured.
All patients were subjected to total lipid profile.
Computed tomographic scan of the brain was performed with the objective of obtaining information regarding anterior circulation ischaemic stroke as the site, volume, shape of infarct. Also, carotid duplex was performed to assess percentage of carotid stenosis and its relation to prognosis of stroke by correlation to NIHSS on admission and mRS one month later.
Comparison between patient group and control group was done as regard NIHSS on admission and mRS one month later.
We found that mRS was statistically higher in patient group than control group.
Mortality was statistically higher in patient group as 20 % (5 patients) of patient group died with p-value (p<0.05) compared to zero in control group.
