Abstract Clipping of the A1 bifurcation aneurysm is considered a critical surgical procedure for neurosurgeons worldwide due to the complexity of the surgical area being dealt with. The present prospective/retrospective study is conducted to discuss the importance of the direction and the geometrical curvatures of the dominant A1 artery and their impact on the choice of side selection of the pterional approach side. 163 patients with ruptured Acom located aneurysms. the aneurysms were treated surgically through a pterional approach. Because of multiple A1 Directions we divided the A1 artery into 2 segments, the first proximal segment which is near the ICA bifurcation, and the second segment which is the distal segment near the Acom-A2 complex. Based on the second segment of the dominant A1 artery direction we categorized the patients into Ascending A1, Descending A1, and Horizontal A1 segments. |
