Abstract Interstital lung disease (ILD) is a heterogenous group of disorders with variable etiologies, clinical presentation, radiographic patterns, and histological appearance that affects the lung parenchyma (the interstitium, airspaces, peripheral airways ,and vessels) ,the common characteristic is the occurrence of inflammatory and fibrous changes . ILD is associated with attacks of acute respiratory failure (ARF) ,Patients with acute exacerbation of ILD can be supported by mechanical ventilation, But this can lead to a condition known as ventilator-associated pneumonia and for improving outcomes in ARF due to some certain causes, noninvasive ventilation (NIV) or nasal high flow oxygen therapy (HFNOT) was studied. Aim: to compare the clinical outcome of Nasal High Flow oxygen Therapy and Non invasive ventilation in patients presented with acute exacerbation of chronic interstitial lung disease complicated with acute respiratory failure and in minimizing risk of invasive ventilation. Subjects and methods: This Prospective randomized study was carried out on 40 patients at Tanta university Hospitals, chest department on 40 patients, from September 2021 to October 2022 , all patients were on conventional oxygen therapy delivered using an oxygen mask, the oxygen setting was set to target SaO2 92,The patients who didn’t respond to conventional oxygen therapy or were intolerant to oxygen mask after 2hours were randomized into 2 equal groups: group 1 (20 patients) will be treated with Nasal High Flow Oxygen Therapy. group 2 (20 patients) will be treated with Non-Invasive Ventilation Results: Rate of mortality and survival equal in both groups but HFNOT is more comfortable and better tolerated than NIV,Furthermore, it is expected to maintain a better quality of life because it easily allows food intake and conversations. |
