Abstract

Introduction: Postoperative pain is one of the primary problems in endodontic treatment. The purpose of this study was to evaluate the effect of occlusal reduction on postoperative pain after endodontic instrumentation in teeth with irreversible pulpitis and symptomatic apical periodontitis. Methods:Fourty-four patients were included in this study. After administration of local anesthesia, the root canals were instrumented.Thepatients were randomly divided into 2 groups of 22 each. In group A, the occlusal surface was reduced whereas in the group B, there was no occlusal reduction. Each patient was asked to to fill the pain scale chart dairy at 6, 12, 24 and 48 hours after root canal instrumentation then at 6 and 12 hours after root canal obturation. Data were analyzed bystudent{uF0A2}s t-test, Chi-square (x2) Mann-Whitney and Wilcoxon signed-rank test using computer program SPSS. Results: from 50 enrolled patients, 44 patients were included in the study. Six patients were excluded because they didn{u2019}t meet the inclusion criteria. group A (occlusion reduction) showed statistically significant difference lower mean VAS scores than group B (no occlusion reduction) at 6 hours (post-instrumentation) and 12 hours (post-instrumentation). Conclusions: Occlusal reduction in teeth with irreversible pulpitis and with tenderness to percussion had significant influence on postoperative pain after root canal preparation only at 6 hours and 12 hours after instrumentation
